Form templates are useful for quickly creating standardized forms for employees to fill out. They save time, ensure consistency, and make it easy to collect structured information for processes like incident reports, inventory checks, or end-of-shift reports. Using templates also helps reduce errors and ensures all necessary details are included every time a form is created. Let's take a closer look!

How to Access and Customize Form Templates
The form template library includes all pre-made forms created by Connecteam, as well as any templates you created. To access form templates:
Access the Forms feature and click on Add New on the top right.
Select the option Use a Template.
At the top of the page, you'll find all the templates you've created, along with pre-made templates organized by industry and topic.
If you would like to filter this list to view only the templates you created, click on the All templates tab on the top left, and select the option Created by me.
To customize an existing template, select the relevant template and click on Select this template on the bottom right.
A new window will open up, where you can make any necessary edits by clicking on the pencil icon next to each field.
Once you're done, click Save!
How to Create a Form Template
Before we begin, feel free to check out this article explaining how to create a new form! After creating your form, to save it as a template so that all admins in the account can distribute it to their departments or teams, follow these steps:
Enter the Forms feature found in the left sidebar and access the form that you would like to save as a template.
Click on Options at the top right of the page, and select Edit
At the bottom right of the page, next to Save, click on Save as Template. In the pop-up window that appears, select Save Template.
💡Tip: If you need to reuse a form you created, there's no need to create a new template, simply duplicate the existing form and make any necessary edits. To duplicate a form, hover over it and click on the Duplicate button on the far right.
Form Template Examples
Form templates can be beneficial if you need to distribute the same form to different departments, or if you need to use the same form regularly, with minor adjustments each time.
Example 1 - Employee Evaluation Form
Suppose your company conducts employee evaluations every quarter. Instead of reusing the same form and mixing entries from different quarters, you can use a form template to distribute a fresh version each time, changing only the date of evaluation. This also enables managers to create separate forms for each team without having to start from scratch every quarter.
Example 2 - End of Job Report
Each department in a company will have different information to report at the end of a shift. A general end-of-shift report template helps keep things consistent while allowing admins to make small adjustments based on their department's needs. Here’s an example:
