Definitely! In Connecteam's Forms feature, you have the ability to create questions that adapt according to your employees' answers. This amazing functionality is known as Conditional Forms. These dynamic forms allow you to create a form that shows the user only the relevant information and fields, based on their answers.

Creating conditional forms begins like creating any other forms. The only difference is that some of the questions will only appear depending on the previous answer.

To create a conditional form follow these steps :

1) Enter the Forms feature

2) Click "Add New" at the top right

3) Select to start a form from scratch or from an existing template

4) Add and edit fields in the form

5) For conditional fields - Add a new field and set a condition for it to show only if a certain answer was selected in the previous field

Take a look at how to create conditional forms from the dashboard below:

If you want to add more complexity to the conditions, you can combine multiple conditions. To do so, follow these steps:

  1. After creating a new field, toggle on the option 'Show this field only if..'

  2. Add the first condition, as detailed in the previous steps.

  3. Click + Add Condition and decide between the options 'and if' or 'or if'.

For instance, in my incident report form, users were asking to describe the severity of the incident. If they marked type of incident as injury, and the severity as 3 (most severe), only then will the field 'Have you notified management' appear.
