Yes, when you export the form entry, there is a way to exclude the name of your employee who filled in the form. The name can be removed if you export all the entries at once to an Excel file or when downloading the PDF entry.

When Exporting the Entries to Excel

Access the Forms feature in the left sidebar, then access the relevant form and click on "Show entries". Click on the table icon on the right in the toolbar, and un-tick First name and Last name or any field you wish to remove. Once you are done adjusting the table with the relevant information, click on "Export" and you will receive the export file with the form entries without the fields you have removed.

When Downloading an Entry as a PDF

Access the Forms feature in the left sidebar, then access the relevant form and click on "Show entries". Hover over the specific form entry you wish to download without the name and click on the arrow icon that will appear. Choose "Download Entry" and under "User fields to include in this PDF" adjust by un-ticking the First and Last name or any field you wish to remove and finally, click on "Download PDF" and you will receive the PDF of the form entry without the fields you have removed.
